Medical Authorization Form
The Medical Authorization Form is available online only.   This form may take about 10 minutes to complete.  Be sure to complete all information for each child attending St. Patrick School.  You will need to provide your child(ren)’s doctor’s contact information, allergy information (if applicable) and emergency contact information.  Please click on the link below to access the Medical Authorization Form.  You will be redirected to a secure site to enter your information.
